Account not getting created after selecting Open New Account from Add Task.

We are trying to create a new account. We have selected “Open New Account” option under Add Task and filled the details as required. After submitting, it says “Open New Account case created successfully and click confirm to continue”. There is a subcase getting created “Display New Account Info (Process New Account Application)” which on opening directly takes to the “Resolve this work” screen where it asks comments for resolving the case and on submitting resolves the case.

Could anyone please suggest what is missing here and how to add a new account?
