It seems that you have missed a few sample data jars that were provided as part of the CS for insurance framework package. Please install all the sample data jars which were mentioned in the installation guide (https://community.pega.com/knowledgebase/documents/pega-customer-service-insurance-and-pega-sales-automation-insurance-83) and check the use-case again.
