Duplicate SOAP request is sent after 60 seconds

We are facing a strange issue in our production environment.

Application Scenario
From our application after all the approvals are done, we have to send a SOAP request to financial posting downstream system. For the past one month we are facing intermittent issues in production environment where the posting is done twice for some of the cases. When I tried to find a pattern for these cases, what i observed is a duplicate SOAP request is sent at 60 seconds. The time out which is mentioned in the SOAP rule is 120 seconds. We could not find the source of this duplicate request which is sent after 60 seconds.

Everytime, we send a SOAP request we add to comments history. In these scenarios where SOAP request is sent twice, the comments history is logged only once. So after troubleshooting a lot now I suspect the there is some flaw or configuration problem with the default SOAP mechanism of Pega where it is sending a duplicate SOAP request at 60 seconds. We are sure, the application code has no defect. The middle ware team also confirmed that the SOAP requests are coming from the Pega server only.

Can someone throw some light what could be the reason for this strange behavior?
