ProblemFlow_WorklistOpenFail Administrator@pega.com

I'm working through the CSA training and all of the sudden, my case flow isn't working. I am getting the following error: ProblemFlow_WorklistOpenFail Administrator@pega.com when advancing to the Manager Review step of my case. Is this something that I configured wrong or is there something wrong with the environment?
