V7.1.5 Portal Display issue

My operators sporadically experience an issue where the Operator gadget displaying their name and the Switch Apps/Portals options is missing along with the Search box also missing. We are running V7.1.5 on IE9. I had the operator clear cache, start a new session, and no better result. It eventually does come back but no explanation as to why it works and then doesn't work. Attached is a shot of the portal and the OOB property used to drive the Operator gadget. Any thoughts?
