Agent isn't executing

I have an activity to cleanup old cases that haven't been updated in X days.  When I manually run the activity it runs fine (I can see that it executed b/c I have log statements in the activity).

I created an agent which calls the activity and I set it up so that it will run every 1 minute (just to make sure it executes).  The agent never calls the activity (no log messages appear that start the activity).

Does anyone have step by step instructions when setting up an agent which calls an activity?  I also tried setting up a Job Scheduler and Queue too.  None of them seem to fire.

Any ideas?  and thanks!
